At Putneymead Group Medical Practice in south-west London, Dr Alison Kirkland sees a woman who blacked out while walking home from work, while Dr Mandeep Baveja tries to discover the root cause of his patient's crippling back pain and inability to control his bladder. Nurse Louise Johnson is doing her best to look after a mother and her three children who were run over by a van, and Dr Kieron Earney and his patient discuss which provides the better service - private health care or the NHS.

Veteran GP Don McKenzie performs a rectal examination, treats a young girl with a marble-sized growth on her forearm, and offers honest advice to a patient who has terminal cancer. Meanwhile, Dr Maria Wallace must bring all of her medical knowledge to bear if she is to correctly diagnose a man whose symptoms include a habit of hitting himself in the face, and Dr Tim King sees a patient with a gum infection that has caused the side of her face to swell up massively.

Dr Ali Hassas sees a man whose alternative practitioner is treating his sore neck by repeatedly punching the painful area while chanting, and Dr Maria Wallace's patient has turned to the internet to restore his flagging libido and has since suffered blackouts and dizzy spells. Dr Devin Gray helps a woman who has been administering a mysterious Chinese ointment to painful blisters.

Dr Lucy Andrews uses all of her experience to diagnose the cause of a teenage boy's mystery illness, and a practice nurse cares for a lung cancer patient who has just weeks to live.

Dr Tim King treats Vittoria, who has been injured during an assault and also has an agonising throat infection, but the real root of her problems is her alcohol dependency and before long the appointment descends into chaos. Dr Ashleigh Helm diagnoses a frightened patient with early onset dementia, while Dr Don McKenzie has a consultation with Harold, a terminal cancer patient who has been told that he has just months to live
